If you go back to Episode select and view all chapters to see if they have a difficulty sign over it showing you beat it on that difficulty. There might be a few that are showing as uncompleted. Had this problem once where two chapters in episode 2 said uncompleted even though I know I beat them. Although you could just try going through them all on easy to make sure. I just went through one chapter last night, not in matchmaking and it unlocked. I didn't do it solo either. A friend was on so we ran though Ep. 1 Ch. 1 on Legendary.
