Back then, if enough people muted you in a set amount of time, your voice would be automatically muted for all other players unless they specifically unmuted you: this is why you'd have games back then with players who you appeared to have muted, even though you hadn't actually done so to them. I think now they just give you an outright ban instead: people may be muting you if you're speaking a language different from theirs, or if they're one of those players who just doesn't like to hear other voices while they're playing.
