    I had the same problem too, after beating the campaign solo on legendary i didnt get the mark iv armor. However, i had a hunch that because i didn't complete one of the missions online it didn't count that mission to the "legend" challenge (I think its this online challenge that unlocks the armor and not the achievement) So I went back and did that one mission again and then it said i had completed the "legend" challenge and the armor was unlocked.
