Since Halo 4 has always had troubles with coop online campaign with other players, even on the 360, it is still most likely the game itself. Not the servers or connection. I know I played Halo 4 on Xbox 360 with friends with my really good connection, I still had a lot of lag and got kicked out. That said, it's just Halo 4.
